The vendor is required to provide that free or reduced fee independent living support services for the delivery of free or reduced fee independent living support services

•Assist older adults ages 60+, and individuals with disabilities ages 18+, with supportive tasks such as:
oPrescription pickup.
oBasic household tasks to support independent living.
oShopping and meal preparation, covering everything required to get a meal on the table, as well as shopping for clothing and other items required for daily life.
oHousecleaning and home maintenance, such as cleaning the kitchen after eating, keeping living space clean and tidy, and keeping up with home maintenance.
oManaging communication, such as the telephone and mail.
oManaging medications, which covers obtaining medications and setting up a process to ensure they are taken as directed.
•Provide relief/ respite for families in which an older adult or individual with a disability resides by providing assisted transportation, companionship, grocery shopping, and other services.
•Provide quality free or discounted services to uninsured and low- income populations.
•Coordinate services provided with family services (DFS), mental health, substance abuse and development services (MHSADS), the area agency on aging (AAA), and other county departments, when individuals are known to receive services from the county.
•Track the following performance metrics:
oNumber of individuals served that are age 60+.
oNumber of individuals served that are age 18+ with a disability
oNumber of referrals from county departments
oNumber of referrals for relief services (such as companionship or transportation).
oNumber of referrals for housecleaning or basic home maintenance or tasks.
oNumber of referrals for food and/or prescription pick-up and delivery.
oNumber/ percent of individuals on waitlist
-All offerors shall:
•Provide comprehensive services for individuals who have developmental disabilities.
•Ensure individuals with limited english proficiency, with limited hearing/ speech, and/ or with low literacy levels can access professional services to assist in understanding procedures and can participate appropriately in decisions about their services or care provision
•Accept individuals for services, regardless of citizenship status or housing status.
•Accept referrals from local agencies or by word of mouth and advertising
•Provide referrals to other community service providers
•Maintain a waiting list for services when demand for services exceeds contracted capacity.
•Participate in regularly scheduled meetings with county staff.
•The executive director of the contracted agency, the contract manager, and the parks, recreation, and community services director or their designee will meet quarterly to review services, trends, gaps, outcomes, and performance.
•The contract manager may conduct unannounced visits with which the contractor must comply
•Make staff available for any county onsite visits as requested.
•Adhere to all relevant federal, state, and local confidentiality and privacy laws and regulations including, but not limited to, the health insurance portability and accountability act (HIPAA) and 42 cfr
•The contractor shall have appropriate administrative, technical, and physical safeguards to ensure the privacy and confidentiality of protected client information.
•Sign and adhere to the county business associate agreement (BAA).
•Retain all working papers, records, and reports, at the contractor’s expense, in compliance with the code of state record retention requirements, unless the contractor is notified in writing by the county of the need to extend the retention period.
•The contractor shall make working papers available, upon request, to the county.
•Maintain confidentiality on all data collected in administration of this contract and in accordance with • Data shall not be released without the prior written permission of the county.
•Maintain appropriate staffing levels to deliver services outlined above.
•All staff must have minimum credentials and licensure as appropriate for their position.
•Complete national criminal and child protective services background checks on staff before their employment.
•Staff working directly with children through this contract must undergo and satisfactorily complete a child protective services background check through the department of social services for, at a minimum, the district in addition to any previous states of residency.
•Performance metrics and reporting: in collaboration with the county, the awarded contractor will collect and analyze data to assess and monitor outcomes, identify and close resource gaps, and drive continuous performance improvement and cost-effectiveness.
•The awarded contractor must enter and maintain additional financial and personnel data, as detailed below, to report progress toward identified performance measures:
oQuarterly program reports outlining the progress of the contractor
oThe reports shall focus on the outcomes and measures to be agreed upon by the county and the contractor as outlined
oAt least seventy-five percent (75%) of its mid-year targets for each performance measure as agreed upon by the county and the contractor by the end of the second quarter.
oThe targets are not met, or the county has concerns about its performance, the county may call a meeting to discuss a corrective action plan with the contractor.
oCorrective action plan must be completed to the county’s satisfaction before additional fund disbursements will be given.
oCorrective action plan consists of a statement of the issue needing improvement and presents the agreed upon remedy, in writing, for completion within a certain timeframe.
oAfter creation of the corrective action plan, the county is responsible for determining when actions have been taken to show improvement and can then approve payments.
oTo meet seventy-five percent (75%) of targeted goals, or goals of corrective action plan, could result in the contractor’s repayment of the awarded funds and non-renewal of the contract.
oAs early as possible, report any programmatic or organizational changes significantly affect the ability to maintain access, quality of care, or the financial viability of the organization.
oProvide a current listing of all organizational and the board of supervisor’s leadership annually at contract renewal.
oTo annual contract renewal, submit the most recent financial statements audited by an independent certified public accountant, the federal tax return, and notification of any financial difficulties or internal control issues that are found that could impair the continuing operability of the organization.
