Professional Services

The vendor is required to provide professional services for location of medical facilities where all services shall be conducted.
- Location shall have laboratory and radiology services available on site.
- Personnel wearing a self-containing breathing apparatus (SCBA) are required baseline medical surveillance examinations, then annual examinations.
- Hazmat medical surveillance examinations:
1. Physical exam and medical / occupational history:
• Lead questionnaire (29 cfr 1910.1025)
• Osha respirator medical questionnaire (29 cfr 1910.134)
• Hazmat baseline medical surveillance examinations
2. Office clinical testing:
• Visual assessment, color blindness, visual acuities (near/far), peripheral fields
• Depth perception – Titus screen
• audiogram
• Resting electrocardiogram (EFG)
• Self-containing breathing apparatus (SCBA) functional assessment
• Spirometry
• EKG
3. Laboratory studies (fasting state preferred):
• Complete blood count (CBC)
• Comprehensive metabolic profile (CMP)
• Lipid profile
• Urinalysis
4. Lifestyle assessment:
• Weight loss issues
• Smoking status
• Exercise status
• Discuss recommend cancer screenings
5. Post examination summary reports:
• Employer receives clearance letter
• Employee receives the personalized letter from visit that includes a copy of laboratory results to review with private physician

• Fasting blood work: complete blood count (CBC), comprehensive metabolic profile (CMP), lipids

5. Lifestyle assessment: weight loss, smoking status, exercise, discuss cancer screenings
6. Post examination summary reports:
• Employer receives respirator and hazmat clearance/exit exam hazmat clearance
• Employee receives the personalized letter from visit that includes a copy of laboratory results to review with private physician
• Audiogram (Exit)
- Additional services, if medically indicated by history or examination:
• Electrocardiogram (EKG)
• Chest x-rays, 1 view or 2 views
• Audiogram
• Spirometry
• Titus vision screen
• SCBA functional assessment
• Tetanus
• Heavy metal urine screen
