The vendor is required to provide to develop a communication and resolution program (CRP) for designated staff members to utilize in the event of an unexpected serious safety event.
1. Conduct a current state validation of city existing policies and processes regarding communication and resolution of serious safety events.
2. Define and develop a sustainable communication and resolution program that will:
• Design a roadmap and methodology for implementing a CRP at city.
• Develop and deliver CRP training to select physician champions and clinical leaders.
• Identify key performance indicators (KPI’S) to track success of the program.
3. Satisfy the requirements of the CMS patient safety structural measures and ensure that the program is Fully implemented by the end of calendar year 2025
- Consultant responsibilities: the successful consultants will:
• Conduct interviews of relevant staff,
• Review policies and procedures,
• Present a gap analysis of city s current behaviors to best practices,
• Create an implementation plan for achieving best practices,
• Create a “train the champion” program,
• Provide all relevant documentation for city to measure and sustain the program.
