The Vendor is required to provide a comprehensive assessment of the county center supportive services and programming.
- Provide recommendations and support services to enhance program effectiveness and improve outcomes for incarcerated individuals transitioning back into the community.
- Program Assessment
• Conduct a thorough review of the current center Supportive Services to include intake, case management, vocational training, education, substance use treatment, mental health services, discharge planning, re-entry and religious supports.
• Evaluate current program structure, staffing, and policies.
• Analyze program data, participant outcomes, and recidivism rates.
• Conduct stakeholder interviews, including staff, formerly incarcerated individuals, and community partners.
- Needs Assessment and Gap Analysis
• Identify gaps in service delivery, resource availability, and community reintegration support.
• Evaluate alignment with evidence-based practices and national reentry standards.
- Recommendations and Strategic Planning
• Develop a written report with actionable recommendations to Supportive Services Programming.
• Identify potential grant opportunities and policy changes.
• Provide a phased implementation plan, including short-term and long-term objectives.
- Technical Assistance and Support
• Assist center with program modifications and implementation support, as requested.
• Provide staff training or facilitation services, if needed.
• Support data collection and performance measurement improvements.
