- The County envisions a solution that facilitates seamless collaboration across departments and external providers by eliminating data silos and enabling secure, real-time data exchange.
- This system must serve as a centralized, interoperable infrastructure that aggregates data from disparate sources—including health, behavioral health, social services, community-based organizations, and managed care plans—and presents a unified view of client and member information.
- The system should also support standard transactions (e.g., 270/271 eligibility inquiries, 837/835 claims processing), and offer advanced reporting, analytics, and consent management tools to inform decision-making, monitor performance, and promote whole-person care.
- capability to integrate into a shared county care team database which aims to house and track members of the client’s care team and their contact information that is accessible via a front-end search tool. Members of the county will have view-only access to enable care coordination.
