The vendor required to provide licensed consulting pharmacist will perform the following services:
• Assist in the development and on-going review of procedures for ordering and storage of medications and the disposal of expired or deteriorated medications.
• Participate in monthly meetings with the medical director, deputy administrator for health services, and facility’s medical staff committee.
• These meetings will also consist of a monthly review of medication errors.
• Separate from the above, the consultant pharmacist will meet with the deputy administrator for health services or a designated representative upon entrance and exit of to the facility.
• Assure that medications are provided in accordance with current standards of professional practice.
• Assist with the development of a system for the appropriate management of adverse drug reactions.
• Be available by phone or fax during normal business hours (8 a.m.-5 p.m., Monday- Friday) for consultation.
• Review each resident’s new admission drug regime.
• Monthly recommendations are to be made to the attending physicians.
• Records of the reviews shall be made to the deputy administrator for health services or medical director.
• The medication administration record (mar) along with the resident’s chart are to be reviewed to insure compliance with mandated guidelines for appropriate drug therapy, avoidance of unnecessary or duplicate therapy, gradual dose reductions (GDR) are being attempted for all psycho- pharmacological drugs or appropriate documentation of clinical contra-indications for a gradual dose reductions (GDR) existsineach resident’s clinical record, and insuring appropriate clinical monitoring of blood pressure, apical pulses, and laboratory values when indicated.
• Shall monitor the ordering of all stock drugs supplied by the nursing home and oversee their storage, control, and dispensing.
• Shall present in-service to the facility staff.
• Shall submit a quarterly written report of the pharmacy committee’s activities to administration.
• Conduct an audit of the doctor's communication sheets for medical records to ensure compliance with pharmacy recommendations.
• Shall perform label changes for medication in which directions have changed.
