o    Black ink, no bleeds
o    Front side only
•    Originals
o    One (1) original design provided by department
•    Artwork supplied
o    Pdf furnished to awarded vendor
o    All state-supplied materials must be returned with proof
2. Quantity, cartooning and labels
•    Initial order quantity
o    200,000 envelopes (400 boxes × 500 each)
•    Carton pack
o    Pack in 500s per carton
•    Required carton labels
o    “BMV 5739 11/25 temporary instruction permit identification card (TIPIC) envelope”
o    Quantity per carton
o    Must include department order number as indicated on p.o., quantity ordered and shipped, requested vs. actual delivery date, material title, and any change order(s)
•    Packing slip

3. Palletization and delivery

o    Fob destination – receiving hours: Monday–Friday, 7:30 a.m.–11:30 a.m. And 12:30 p.m.– 3:00 p.m., excluding state holidays.
