The vendor is required to provide to establish and utilize a performance management model with quality improvement practices and processes in an electronic system solution that supports continuous improvement of the agency’s practices, programs, and interventions.

1. Mandatory requirements:
• Provide for single sign-on (SSO) using the state's identity service.
• The ability to provide a dashboard or scorecard to report results
• The ability to provide a cloud-based system solution.
• The ability to provide managed services with ongoing remote information technology support.
• The ability to collect and analyze performance data.
2. Preferred requirements:
• The capacity to support a statewide approach to include adoption by local health departments.
- Data collection and reporting feature requirements
• The ability to fulfill board requirements of implementing a performance measurement system.
• The ability to collect survey data and simplify data collection and reporting processes.
• The ability to monitor progress toward goals by systematically collecting data and display via system dashboard.
• The capacity to create linkage and data aggregations across the state’s local partners.
- Provides a project work plan which includes:
• Includes a detailed timeline and milestones for training.
• Training delivery options.
• Performance management tools
• System build-out timelines.
- Provides virtually, in-person, and recorded training as well as system build-out details to department ensure successful implementation of the performance management system that results in a well-developed monitoring system that includes:
• Activities completed.
• Performance outcomes based on the quality and success of implemented interventions.
• Whether the intervention led to measurable improvements in the targeted outcomes.
