- PH EHR solution as a system that houses electronic versions of client medical records and case management information, is maintained by designated providers over time, and may include all key administrative, clinical, and case management data relevant to the clients’ care under a particular set of providers, including, but not limited to, demographics, progress notes, problems, medications, vital signs, medical history, immunizations, laboratory data, and radiology reports.
