Family Assessment Form Replacement System

The vendor is required to provide family assessment form (FAF) replacement assessment tool. 
- The FAF replacement system must capture all needed documentation in order to accurately reflect the delivery of contracted services by community-based agencies who provide family preservation services to families, children and youth in county. 
- Required documentation includes comprehensive case management services, family assessments, contact notes of services rendered, monthly progress reports, and case plans. 
- This application must also be able to gather necessary information to help inform program usage as it pertains to services rendered, as well as program evaluation and outcomes. 
- This includes tracking demographics served, number of program referrals, utilization and expenditure reports, completion timelines of required documentation, and program data reports.
- Contractor will develop, implement, and maintain a reliable custom web-based FAF replacement system for the family preservation program (FPP).
- Department project team, which will consist of division administration, program leads, and business information system (BIS) technology specialists.
•    Contractor will work with bis technology specialists to support the data migration of department existing documentation system into the newly designed system.
•    The software system with ongoing input and feedback from division regarding the needs of the programs.
•    Develop user manuals and training video tutorials in collaboration with the department project team to assist with training and implementation of the customized web-based documentation software system.
•    Provide ongoing maintenance and technical support required to troubleshoot any identified problems during usage of the software system.
•    Responsible for the installation and configuration of the application and its database on department dedicated hosting servers on azure SQL.
•    Safeguard clients’ personal health information (PHI) and personal identifiable information (PII) as outlined in the information security and privacy requirements and must comply with insurance requirements.
