Provider Incentive Platform

The Vendor is required to provide for to develop a physician incentive program.
- The program will provide primary care providers and specialists with incentives to engage in member outreach. 
- The incentive program will include provision of an online platform, implementation, and training and post-implementation support.
- County residents who are eligible for medical and OneCare, which includes families with children, seniors, persons with disabilities, foster care children, pregnant women, and low-income people with specific diseases.
- Desires to shift from a retrospective ‘pay for performance’ model to a real-time, point-of-care approach that rewards physicians for the completion of specific value-based care actions. 
- Develop a tailored incentive software system and incentive program that can be implemented rapidly.
- Recruit and train providers and office staff to use the platform through in-person and virtual meetings. 
- Recruitment will include training in platform functionality and pay-for-value program components.
- Provide physicians and office staff support for answering questions, troubleshooting, and managing system downtime or errors.
- Define measurable performance thresholds and incentive payment methodologies (e.g., per-member/per-gap closed, percentile ranking, and improvement over baseline).
- Provide insight into measure-level or member-level incentive amounts (e.g. $20 per Colorectal Cancer Screening gap closed).
- Provide measure-level performance dashboards and /or scorecards to providers that are refreshed timely (minimum of monthly).
- Blended approach (i.e. a combination that allows for special emphasis on components of the program while holding providers accountable for overall performance).
- Customizable payment distribution models, e.g. earnings can be shared amongst staff and physicians at the practice.
