The Vendor is required to provide for the provision, implementation, configuration, support, maintenance, and ongoing operation of a health monitoring software and device solution to support remote monitoring of patients in the community.
- Solution shall enable agency to remotely monitor patient health status between in-person interactions, support chronic patient populations, assist with symptom management and treatment oversight, and provide timely notification of acute deterioration and progressive clinical changes over time.
- The solution must have tools and solutions to monitor for changes, exacerbations, and progression of disease states including monitoring devices specific to patient chronic conditions such as, but not limited to glucometers, pulse oximeters, weigh scales, and blood pressure machines.
- Alternative monitoring devices may also be necessary.
- The solution must also include a secure, customizable, care pathway platform that generates automated alerts and ensures reliable connectivity between patients, caregivers, and clinicians.
- Provide a complete, operational, and supported solution including all software, devices, peripherals, firmware, implementation services, support services, maintenance services, training, documentation, and related deliverables required for agency to deploy and use the solution.
- The platform shall include:
• A web-based (cloud-based) portal for agency users;
• A patient-facing interface with simple and intuitive workflows;
• Patient file and record management capabilities;
• Administrative dashboard functionality;
• Configurable alerts, alarms, and clinical decision support functionality;
• Standard reporting functionality; and
• The ability to support agency operational workflows for patient monitoring and follow-up.
- Patient-Facing Requirements
• Enable patients to record and transmit health values, vital signs, and other configured monitoring information;
• Be easy to use and navigate;
• Provide a clear and simple process for patient data entry;
• Support reminders or prompts to complete readings or scheduled monitoring activities, where available;
• Provide access to patient education materials, tutorials, and illness self-management resources, where available; and
• Support notification workflows for acute changes and progressive deterioration, where configured by agency.
- Devices and Peripherals
• Blood pressure monitor(s);

• Additional monitoring devices, where clinically applicable.
- Clinical and Administrative Portal
• Secure user access for authorized personnel;
• Patient/client lookup;
