The vendor is required to provide case management services for the state fund, transition in place (TIP) program.
- These services shall be used on as needed, if needed basis.
- The payment structure for case management services can vary depending on the number of veterans approved for the TIP program.
- The TIP program awards up to $2,000 for case management services and up to $7,000 for housing support services for each veteran.
- Have the knowledge of the local resources and ability to connect with well-known established housing entities and provide case management for homeless veterans.
- Maintain and utilize a policy and procedure that includes, at a minimum, detailed intake procedures, program description and eligibility requirements, nondiscrimination policy and confidentiality requirements.
- Verify and ensure that staff members do not have conflict of interest in the provision of case management services.
- Such as veteran status, relatives or personal interest.
- Utilized a computer-based tracking system to generate reports on a monthly and quarterly basis, and any other reports as requested by advs.
- Provide all staff applicable training for services provided by the contractor in a timely and accurate fashion.
- Identify and address social determinants of the veterans enrolled in case management to include but not limited to: housing, food stability, psychological care, social support systems, spirituality, etc.
- Provide services with one (1) full-time dedicated case manager for every fifteen (15) homeless veterans to assist with connection to housing, military resources and wrap around support.
• Provide additional case managers if the number of homeless veterans increases past fifteen (15) program participants.
- Agency must be notified of all requests for termination of individual case management within twenty-four hours (24).
- Veteran engages with case management provider;
- Case management provider attends housing intake/orientation with the veteran at the housing provider’s offices to start housing assistance process;
- Case management provider must address the needs and/or barriers identified by the veteran and if applicable, coordinate care plans with veterans’ affairs (VA) social worker; and
- Case management provider helps veterans with finding employment or enrolling in higher education, and provides employment referrals and/or resources through the VA.
