Case Management Services

The vendor is required to provide intensive case management and wrap around services to assist in the implementation of a pilot project to provide emergency shelter and rapidly rehouse homeless youth (the project participants).
• Individualized case management
• Housing location assistance
• Stabilization services
• Participate in bi-weekly team meetings
• Execute master lease agreements when deemed necessary.
