Child Study Evaluation and Case Management and Testing Services

The vendor is required to provide a comprehensive independent child study team evaluation on children referred on an “as needed” basis.
- The dominant language of the referred child and shall include the following:
• Psychological examination
• Educational assessment
• Social assessment
• Comprehensive health appraisal
• Case management
• IEP management
• Initial evaluation and referrals as needed
• Eligibility management
• Review IEP and re-evaluate
• 3-5 days a week
• Attend required meetings
- The contractor shall have access to, and the district shall make available, copies of all previous evaluations, when necessary, in the performance of the contractor’s services.
- Contract Period/Term: 5 years
