The vendor is required to provide the targeted case management and crisis case management program is a community-based model that will serve individuals who are living with a mental illness and/or substance abuse and are in need of structured supports, guidance, and rehabilitative services to maintain stability in the community.
- The case management program helps individuals increase their independence in the community and live with dignity.
- Facilities, adult family homes, community support programs, comprehensive community services, and targeted case management.
- The case management program offers a pathway to an independent and fulfilling life for individuals living with a mental illness who are being served in the behavioral health system.
- Targeted case management (TCM) is a low intensity co-occurring case management model.
- TCM includes a primary case manager model with a peer specialist service.
- Crisis care management (CCM) is an intensive case management model.
- CCM utilizes a team approach to stabilize the crisis occurring in the individual’s life.
- The team may consist of a supervisor, clinician, case manager, peer specialist, and crisis stabilizer.
- A case management provider will be engaging and assessing the consumer’s needs to provide services.
- When the consumer is placed in TCM level of care, they will be provided assistance with community functioning from a case manager for as little as twice a month up to once a week.
- If a consumer needs more intensive services, then they are a candidate for crisis services or another level of care.
- The consumer will start receiving more services under the crisis care management episode until the crisis has been resolved.
- Crisis care management can be delivered based on the needs of the consumer and can be as little as twice a week to several times a day.
- If the consumer has been receiving crisis services for 4 months there will be a staffing to discuss next steps.
- Services in both programs include face to face home visits, phone calls, emails, telehealth, application process for community services, medicaid enrollment process and updates, collateral contacts, and payee services if needed.
- Both programs follow the policy and procedures created for consumer care.
