The vendor is required to provide case management and program services which will engage up to 600 mandatory participants in county and achieve the goals and objectives of the works program.
- Case management and service coordination
• Case management is a concentrated and comprehensive approach to helping participants secure and maintain employment through the delivery and management of services reflecting diversity and cultural sensitivity.
• The assistance of the case manager, the participant will develop an individual plan (IP) based on the assessments, outlining the steps necessary to secure employment and achieve economic self-sufficiency.
• The case manager will coordinate the services provided by all service providers involved with a participant’s IP.
• The case manager shall continually monitor the participation and progress of the participant.
• It is the case manager’s responsibility to help the participant identify and eliminate any barriers affecting their ability to participate effectively in their IP.
• Ensure planned services are made available to the participants.
• Help the participant to obtain and maintain employment through the delivery, coordination and management of services.
• Provide for any counseling and services determined to be appropriate for the participant’s successful achievement of goals identified in their IP.
• Involve the participant in establishing and meeting the time frames and benchmarks built into their ip.
• Focus on career and life plans, rather than just getting a job.
• Provide concentrated time with individual participants to establish goals, coordinate providers of service, monitor progress, collect documentation, etc.
• Establish an environment of trust and partnership.
a. Contractor responsibilities include but are not limited to:
• Provide works program orientation to participants.
• Be knowledgeable of and apply works federal, state and county rules and regulations.
• Complete the works screening and assessment to determine the participant’s specific strengths and needs and to gather information necessary to provide a framework for the development of their IP
• Develop an IP with the participants.
• Facilitate the participant’s immediate, full and continued engagement in their contracted activities.
• Be knowledgeable about all community resources to assist the participant.
• Monitor and document attendance, participation and progress.
• Obtain feedback from the participant and other collaborating entities.
• Follow up with the participant on a regular basis and as events occur.
• Initiate appropriate and timely contact with county human services staff.
• Complete ongoing assessment of the participant’s progress and make adjustments to the IP as determined necessary.
• Explore and incorporate options to maximize the participant’s success.
• Maintain a written record of each participant’s communications, participation, progress and status.
• Facilitate the conciliation and demonstrable evidence process.
• Attend mediation and fair hearings as required
• Ensure participants meet federal participation rates.
• Notify department community support services (CSS) division when a participant has failed or refused to participate without good cause.
• Complete all required reporting to department CSS division.
- Unsubsidized employment
• Employment may be full or part-time in the public or private sector and is not subsidized by agency or any other public program.
• Employment must consist of work for pay.
• Pay must not be less than either the federal minimum wage or the state’s minimum wage, whichever is higher.
- Community service
• Work site placements should take into account the participant’s interest and skills if possible.
• Provide pre-employment and work maturity skill training as determined necessary prior to work site placement.
• Work site development.
• Develop community service agreement with the work site.
• Conduct work site communication.
• Perform work site monitoring and completion of progress notes and attendance records.
