The Vendor is required to provide participants in the family self-sufficiency program (“participants”) through a combination of individualized professional social work assistance and an educational group format to guide low-income families to self-sufficiency.
- Orientation, Intake and Assessment
• The Case Manager shall engage the Family Self Sufficiency Program applicant in a worker/participant relationship.
• The case manager shall provide orientation for the participant that includes an overview of the family self-sufficiency program.
• The case manager shall elicit the participant’s expectation of services, discuss the function of the self-sufficiency program, and explain the roles of both the case manager and the participant.
• The case manager shall explain the contract of participation and emphasize the participant’s rights and responsibilities to the participant.
• The case manager shall also explain the guidelines for frequency of contact with the case manager.
• The case manager shall complete an intake and assessment during or after each meeting with the participant.
• While meeting with the participant, the case manager shall elicit pertinent personal data from the participant to assist the case manager in identifying personal areas of strength and stress; and identifying environmental, psychological, social, and cultural variables impacting the participant, especially those variables that form barriers to a participant’s journey towards self-sufficiency.
- Contract of participation/service plan
• The case manager shall mutually establish goals and develop a service plan with the participant by using a strength-based perspective (the “service plan”).
• The case manager shall provide positive reinforcement and emotional support for the participant.
• The case manager shall prepare a written contract of participation that includes the service plan containing the participant’s long and short-term goals toward self-sufficiency.
• The contract of participation shall outline appropriate tasks for the participant to follow through for goal achievement.
• The contract of participation shall be signed by both parties and a copy retained by the participant.
• The case manager shall meet with the participant at least once every six months.
• At each visit the case manager shall bring the written contract of participation/service plan and review the progress the participant is making towards self-sufficiency.
• Depending on the participant’s progress, necessary amendments shall be made to the contract of participation.
• After the case manager engages the participant in an assessment and develops a written service plan, the case manager shall conduct or arrange follow-up services.
• These services shall include, but are not limited to, advocacy, referrals, entitlement assistance and crisis intervention.
• Once the follow-up services are arranged, the case manager shall follow-up with the referral agency to determine the level of the participant’s cooperation and participation with the services and shall record such cooperation and participation in the participant’s file.
• All exchanges of information require a confidential release of information form signed by the participant.
• The case manager shall track the participants to ensure compliance with the program.
• The case manager shall maintain all files and statistics for the preparation of monthly and/or quarterly activity reports, which shall be transmitted to authority.
