Case Management Plus Services

The vendor is required to provide that case management plus services for include:
1. intensive case management (ICM)-plus case management services, include:
a. Case management
•    ICM-plus high utilizer*
•    ICM-plus homeless*
b. Psychosocial rehabilitation services, including:
•    Health and wellness related meetings, groups and activities
•    Living skills, meetings, groups and activities
•    Recovery oriented meetings, groups and activities
c. Treatment services, including:
•    Substance use therapy for those with co-occurring mental illness and substance use disorder (COD)
•    Group psychotherapy
•    Individual psychotherapy
•    Medication management
•    Cognitive behavioral therapy
•    Motivational interviewing
•    Contingency management principles 
•    Nursing
d. Certified peer support services, including: 
•    Peer co-facilitated groups
•    Peer-to-peer supportive counseling
•    Self-management meetings
2. These types of services are delivered in a clinical sequence that includes:
•    Intake and consumer orientation
•    Team collaboration, liaison, and advocacy
•    Individualized recovery planning
•    Crisis planning
•    Clinical services, including:
o    Helping the consumer to obtain entitlements, benefits, and housing
o    Working with agency to address forensic services 
o    Integrating services that address co-morbidity
o    Integrated cod services
o    Natural supports
o    Peer support services
o    Psychiatric treatments
o    Psychosocial rehabilitation (PSR) skill-building services
o    Symptom assessment and management
•    Evaluation
•    Service changes, transfer, or discharge.
