The vendor is required to provide post release case management that aims to support individuals who demonstrate high need and high risk upon release from prison, have a primary substance use disorder, and currently receiving medication assisted treatment.
- The post release case manager (PRCM) will operate as a source of support, providing brief assessment of reentry needs, establishing reentry goals, and measure reentry progress across a period of time.
- The PRCM will establish frequent meetings with the reentrant soon after release progressively decreasing as the reentrant shows progress and growth in managing intrapersonal needs and general life tasks that support their overall recovery.
- The risk for recidivism is likely to be reduced as the presence of healthy social behaviors increase.
- PRCM will work with each client to create a care plan that addresses the client’s immediate post-release needs at the moment of discharge and address the long-term transitional period in the community.
- Post release case manager duties
• The PRCM will utilize an evidence-informed case management model such as the clinical case management model and the strengths-based clinical case management model that supports provision of direct services to support acquiring resources to maintain stability, advocacy, and assist in developing informal support systems.
• The PRCM shall incorporate motivational interviewing techniques and cognitive-behavioral interventions to support their selected style of case management.
• Collaborate with bureau of substance abuse, mental health (BHSAMH) director of reentry services, level v and level iv healthcare provider of services and security staff prior to client’s release from prison to attain details of former.
• Collaborate with doc bureau of community corrections.
• Collaborate with center to ensure compliance with court requirements.
• Maintain a caseload of 30 clients.
• Enroll and utilize the state treatment and referral network to initiate and track referrals to community providers throughout the case management period for each client.
• Complete biopsychosocial evaluations for clients prior to release.
• Assist with scheduling behavioral health, medical and social service appointments (i.e., social security disability insurance and supplemental security income).
• Encourage clients to follow through with mental health and substance use treatment community appointments.
• Demonstrate knowledge of community supports to include but not inclusive of: housing supports, vocational and training resources, family support services, substance use recovery programs to encourage effective problem-solving and increase practice of positive engagement.
• Maintain case documentation including reentry needs assessment, evaluation outcomes, case activity, case management sessions, recovery plan and recovery plan progress.
• Conduct routine face-to-face contact with clients for monitoring progress with transitioning, progress in treatment services, progress with life-skills activities (i.e., setting and making appointments, parenting, employment, housing, prosocial activities, prosocial associates.) refer to monitoring section for additional details on client contact requirements.
• Conduct bi-weekly conference meetings with BHSAMH to provide update on service delivery.
- Case management and encounters
• PRCM shall review client’s current reentry needs assessment and reentry plan.
• PRCM shall make contact with client’s institutional reentry specialist and primary treatment clinician to discuss client’s treatment summary prior to release.
• The PRCM will meet with the client within three days following their release to finalize initial recovery plan with clear and measurable objectives to prepare the client for independent management of recovery.
- PRCM reporting requirements
• The PRCM will provide a weekly report to BHSAMH social service administrator and behavioral health treatment services director reflecting the number of active cases, discharged cases, reincarcerations, violation cases, reincarcerations, Narcan events, inpatient stays, new admissions.
• The PRCM will provide a monthly summary report capturing the progress of each client.
• The PRCM will offer a satisfaction survey to each client prior to conclusion of case management services.
