The Vendor is required to provide to conduct a program evaluation related to patient referral pathways and clinical case management services for residents in agency.
- The specialists book accommodations and travel for the client.
- In Iqaluit, referrals are generated by a physician or nurse practitioner, with Iqaluit Case Management supporting the process.
- This will involve examination and analysis of each of the current programs, services, human resources, structures, and pathways in each of the three regions including:
• Total volume of medical travel and medevacs per region over the last 5 years
• Breakdown of categories of medical travel and reason for medevac per region over the last 5 years.
• Projections of future volume for each program over the next 5 years.
• Detailed mapping of current systems for case management and patient referral programs including discharge instructions, diagnostic results and return to state follow up care, by region.
• Current resources - human, financial, technological - associated with case management and patient referral programs in each region and anticipated future demand.
• Clinical and administrative documentation and information systems.
• Program administration, oversight, and continuous quality improvement management.
• Program data collection, management, analysis, and reporting.
• Review of program policies and procedures.
• Focus groups and key informant interviews.
• Use of external organizations/partnerships.
• Identification of program pain points, gaps, areas of deficiency, areas of duplication, resource intensive areas, inefficiencies, etc.
• Generate a visual map of current processes and workflow for case management and patient referral programs in each of the three regions.
- Review other models, standards, and practices in provincial/territorial jurisdictions with a special focus on northern, remote, and rural jurisdictions.
- Review promising/leading/best practices, trends, national standards, and innovative solutions for providing streamlined case management and patient referral programs, ensuring the review is sensitive to state unique challenges.
