The Vendor is required to provide to conduct evaluation of case management practices.
- Staff to plan and execute evaluation projects, addressing the following aims:
o Assess the implementation of the existing department case management toolkit,
o Assess the usage of department participant tracking system for case management purposes,
o Examine variation between and within department program areas, and
o Gather input from program providers about the case management tools provided by department or other frameworks they may be using.
- This evaluation will be used to evaluate current case management models, inform future program design efforts and help determine the types of resources needed to promote quality programming and case management care.
- As participants prepare to exit services, staff collaborate on follow-up plans that reinforce stability and connect individuals to longer-term supports.
- This structured documentation supports compliance while maintaining a focus on the participant’s evolving priorities. Reassessments occur at least every three months, or more often as needed, to account for changing circumstances.
