Inmate Health Care Services

The vendor required to provide to operate the health care program with adequate staffing for no less than 24 hours per day, seven (7) days per week, 365 days a year.
- Medical staff:
•    Provider must recruit, interview, hire, train, and supervise all health care staff and said health care staff must be adequate to meet all conditions and specifications of this contract.
•    All medical staff providing services under this contract must be licensed to practice in the state.
•    One (1) full-time, on-site health care administrator shall be provided who will have general responsibility and knowledge of operations and facility for the successful delivery of health care for the county.
•    One (1) 30-hour per week, on-site medical doctor.
- Receiving and screening:
•    Current illnesses and health problems including those specific to females;
•    Medications taken and special health requirements;
•    Screening of other health problems designated by the responsible physician;
•    Behavioral observation, including state of consciousness and mental status;
•    Notation of body deformities, trauma markings, bruises, lesions, eye movement, jaundice;
•    Condition of skin, including rashes and infestations;
•    Disposition, if applicable;
•    Document referral of prisoners to qualified medical personnel for emergency treatment;
•    Notation of personal physician and medical needs; and
•    Assessment of suicidal risk.
- Screening:
•    Current illness and health problems including mental, dental and communicable diseases; 
•    Medications taken and special health requirements; 
•    Alcohol and drug use, including types, methods, amount, frequency, date or times of last use and history of problems related to stoppage. 
•    For females, a history of gynecological problems and pregnancies; 
•    Behavior, including state of consciousness, mental status, appearance, conduct, tremors, sweating; 
•    Notation of body deformities, trauma, markings, ease of movement;
•    Condition of skin and body orifices, including rashes and infestations, needle marks, or other indications of drug abuse.
- Health appraisal:
•    Each inmate held for 72 or more hours shall be given a health appraisal, including a physical examination by qualified health care personnel within 14 days after admission to the facility.
•    Review of intake screening forms;
•    Collection of additional data regarding complete medical, dental, and psychiatric histories;
•    Appropriate laboratory and diagnostic tests to detect suspected communicable diseases;
•    Documentation of height, weight, pulse, blood pressure, temperature;
•    Physical examination with comments about mental and dental status;
•    Gynecological assessment must be included for females;
•    Physician review of physical examination and test results for problem identification;
•    Initiation of therapy as medically indicated;
•    Other tests and examinations as appropriate and indicated, including but not limited to, pregnancy tests, communicable disease screening, HIV screening, and chest x-rays.
- Health assessment of females must include documentation of the following:
•    Menstrual cycle and unusual bleeding, the current use of contraceptive medications, the presence of an IUD, breast masses and nipple discharge, and possible pregnancy;
•    Pelvic and breast examination and pap smear initiated when medically indicated by the physician.
•    Abnormal results of the health appraisal shall be reviewed by a physician for appropriate disposition. 
•    Health assessment of federal prisoners and ice detainees must include all of the above plus verification and administration within 24 hours of intake.
- Nursing services:
•    Routine nursing services will be required 24 hours per day, seven (7) days per week.
- Sick call:
•    Routing diagnosis and treatment of minor health problems will be handled through a sick call system. 
•    Sick call shall be held seven (7) days per week.
- Clinic:
•    A physician on call 24 hours per day.
•    Supervision of the clinic by a charge nurse on a daily basis, must be a RN, when the healthcare administrator is not available. 
•    Daily nursing services, with a minimum of three (3) LPNs, or above, on duty to adequately cover the inmate population. 
•    A manual of nursing care procedures. 
•    A separate and complete medical record for each inmate. 
•    Electronic health records are required.
- Medical records:
•    All inmate medical records must be always kept up to date. 
•    The record shall accompany the inmates at all health encounters. 
•    Pertinent information must be forwarded to the gaining facility in the event of inmate transfer. 
•    Access to medical and dental records must be always controlled by health care personnel. 
•    All rights concerning the confidentiality of the medical records must be followed. 
•    The transcription and filing of information in medical and dental records will be done by professional nurses and trained medical records clerks. 
•    Under no circumstance is any inmate allowed access to medical, mental health and dental records. 
•    The provider shall give the designee access to records on a need-to-know basis, and provide copies, consistent with confidentiality laws upon request.
