Chronic Disease Prevention and Health Promotion Services

The Vendor is required to provide to advance the chronic disease prevention and health promotion (CDPHP) unit of the division’s mission to address leading causes of death, injury, and disability, with a specific focus on chronic disease prevention and positive youth development.
- The primary goal is to address health equity by implementing environmental, policy, and systems strategies that improve access to culturally relevant, healthy food options, celebrate and preserve culture to foster social cohesion.
- This partner is responsible for the overall execution of the strategies, reporting, market operations, and community engagement
- Integrated Health Promotion and Prevention: Focusing on integrated chronic disease and health promotion prevention, including substance use prevention (i.e., nicotine, tobacco, fentanyl, alcohol) and injury and violence prevention focused on traffic, bike and pedestrian safety, Crime Prevention through Environmental Design (CPTED), Pride, Peace and Prevention (PPP), social cohesion, and community building.
- Cultural Preservation and Community Cohesion: Planning, coordinating, and executing the county annual celebrations and implementing health promotion programming that supports community outreach and engagement, promotes social cohesion and community building, and preserves the cultural heritage of residents impacted by displacement and gentrification to foster a sense of belonging in East County.
- Food Systems and Infrastructure Development: Operating and maintaining essential community assets by implementing the 15-week Sunday People’s Farmers Market (including culturally specific food access, supporting logistics, and coordinating produce prescription and incentive programs). This also involves leveraging existing relationships and platforms to provide culturally specific education content on food, community clinical linkage, and built environment related topics, alongside demonstrations, leading active transportation initiatives, and identifying policy, systems, and environmental changes.
