Employee Verification Services

The vendor required to provide employee verification services for employee information, employer, employer identification number (EIN), employment status, most recent start date and termination date (if applicable), total time with employer, job title, rate of pay, employee pay frequency, average hours per pay period, pay cycle, income summary, and pay period detail.
- Requirement:
•    Employee information: customer's full name and social security number.
•    Employer: name, address, EIN, and wage garnishment address.
•    Employment status: status of employment (e.g., active, inactive).
•    Employment dates: most recent start date and termination date (if applicable).
•    Job details: total time with employer and job title.
•    Pay details:
o    Rate of pay
o    Pay frequency (e.g., weekly, bi-weekly)
o    Average hours per pay period
o    Pay cycle
•    Income summary: up to three (3) years of year-to-date gross income details.
•    Pay period detail: up to three (3) years of detailed pay period information.
•    Benefits: medical and dental insurance information.
•    Other: workers’ compensation information.
