Life and AD and D Services

The Vendor is required to provide currently offers Basic Life/AD&D and Voluntary life for employee, spouse, and children.
- Current Benefits:
•    Basic Life/AD&D: 
o    1x BAE to $150,000 max rounded to the next higher $1,000 if not already $1,000.
•    Voluntary Life for Employee: 
o    $10,000 increments from $10,000-$150,000 Guaranteed Issue
o    Maximum $500,000
o    50% reduction on policy anniversary date following age 70
•    Voluntary Life for Spouse: 
o    $5,000 increments from $5,000-$25,000 Guaranteed Issue
o    Maximum $250,000
•    Voluntary Life for Children: 
o    $5,000 increments from $5,000-$10,000
o    Child defined as 15 days to age 18 (older if disabled).
