Core Free Medical Services

The vendor is required to provide critical direct care services in the areas of core free medical services, to vulnerable populations with a record of high-quality performance and organizational and fiscal stability.
- Provide medical services included:
• Provide free primary and specialty care services to patients.
• Provide access to free laboratory and imaging services.
• Provide free medications.
• Provide free care coordination.
• Provide free case management.
• Provide collaborative care and medication for general/situational behavioral health needs.
• Provide referrals to a comprehensive medical home for eligible patients.
• Track the following Performance Metrics.
1. Number/percent of patients receiving an evening appointment with a two-week period.
2. Number/percent of patients receiving a flu vaccine.
3. Number/percent of no-shows for appointments.
4. Number of referrals to comprehensive medical home.
5. Number/percent of sick patients seen within two (2) business days of being contacted.
6. Number of patients receiving referrals for behavioral health needs.
- Provide comprehensive services for individuals who have developmental disabilities.
- Ensure individuals with limited English proficiency, with limited hearing/ speech, and / or with low literacy levels can access professional services to assist in understanding procedures and can participate appropriately in decisions about their services or care provision.
- Accept individuals for services, regardless of citizenship status or housing status.
- Accept referrals from local agencies or by word of mouth and advertising.
- Provide referrals to other community service providers.
- Maintain a waiting list for services when demand for services exceeds contracted capacity.
- Maintain appropriate staffing levels to deliver services outlined above. All staff must have minimum credentials and licensure as appropriate for their position.
