Online Survey Services

The vendor is required to provide that online survey services
- Seeking a vendor that can provide a web-based hosted application 360 tool for professional development of its leadership team by incorporating multiple perspectives using feedback from a variety of sources.
- Sources may include supervisors, peers, subordinates, self, and internal/external stakeholders.
- The vendor's web-based hosted solution shall include but may not be limited to provide:
• Anonymous logging into the survey website by participants.
• E-mail invitations to the survey as well as an e-mail reminder once the survey is initiated by the agency administrator.
• Preloaded standard customizable questions that are associated with a 360 review.
• Customization to allow agency administrator add or delete questions from the survey.
• Customization features to allow a agency administrator to incorporate questions developed by agency into the survey.
• Surveys shall have at a minimum of 100 questions.
• Surveys of each category must include a "comments" section that will read comments verbatim in order to maintain anonymity of the participant.
• An "additional comments" section at the end of the survey.
• Each survey must be able to have up to 140 participants including the survey target.
• Participants access the survey from any computer with internet access.
• Agency administrator will have immediate access to the report that is associated with that survey, upon completion of that survey.
• A feature to utilize a customized rating scale.
• A feature to modify participant status
• Survey report should include radar chart, and overall rating.
- Additional requirements:
• Data collected during contract will not be shared under any circumstances with any third party.
• Any information gathered will be the property of agency.
• The application shall not have to interface with agency information technology (it), specialized software, or any specialized equipment.
• Agency shall provide e-mail addresses which the vendor will be responsible for preloading into the system to allow for easy drop down and/or autocomplete.  
• Vendor's solution shall provide ready to use templates for reports and customizable features for reports.
• Reports must be in pdf format
- Contract Period/Term: 4 years and 5 months
