The Vendor is required to provide for pharmacy services for long-term care home.
- Clinical Collaboration
• Describe on-site medication management process for each resident’s medications
• Provide consultation for the development of pharmacy related policies and procedures
• Participate as a member of the interdisciplinary facility team during path of life meetings/care conferences and medication reviews
• active communication methods with the healthcare team to ensure safety and health of the residents and staff
- This is a multi-year journey, requiring the support and involvement of patients, health care providers and partners across the health system.
- Working together to better co-ordinate health services across the province to ensure you receive the right care, in the right place, at the right time, and by the right provider, wherever you live in state.
- Billing procedures that would be used – frequency, responsibility to collect, credit process for returned medications that can be re-dispensed, sample bill, costs for repackaging, and any additional charges
- Ability to comply with authority electronic requirements
- System to be used, also include dosage system for PRN and multi-dose medications
- Define hours when pharmacy services would regularly be available and method of obtaining after-hour emergency services.
- Provide onsite medication education for residents, staff, and families
- Identify process for delivery of narcotics and controlled drugs
- Identify process for managing pass medications
- Provide sample documentation or demonstration of electronic medication administration record
- Provide availability to participate in local, network and provincial committees
- Optimize medication management to improve health outcomes for residents
- Additional services that will be provided to the resident
- Define the comprehensive medication management process
- Define process for the confidential disposal/destruction of medications and confidential disposal of the packaging in the case of patient identifiers present.
- Process for transition of resident information in the context of transition of services either with a change of location of resident or a change of contracted provider.
- The medication reconciliation process at transitions of care
- Complaint management process for residents.
- Include Med Pouch disposal, Stat Box/Emergency Box listing.
- Contract Period/Term: 3 years
