Employee Background Screening Service

The Vendor is required to provide employee background screening services for the agency.
- Provide professional background screening services to support agency hiring and workforce onboarding processes.
- Criminal History Report. Report criminal history per location where applicant resided (i.e. county, state, and federal, as requested, for all names under which the applicant has worked for all areas of residency per the employment application for the last seven (7) years. Conduct criminal history check for areas of residency not provided by applicant as evidenced by social security number trace, if applicable. Criminal history reports regarding convictions shall include: (i) case number, (ii) charge/offense, (iii) disposition date, and (iv) sentence-trace reports.
- Social Media screening.
- Cross-check of sex offender lists (state and federal).
- Cross-check of child abuse registry.
- Verification of Social Security Number (SSN). Verify that the SSN reported by the applicant is valid. The SSN verification report shall also provide the current or prior address information not listed by the applicant as well as any aliases.
- Employment Verification: Verify applicant's current and previous employers and work experience for the most recent ten (10) year work history for all names under which the applicant has worked.
- Education Verification: Confirm applicant's level of education as reported on application. The report shall include: education diplomas, degrees, number of college hours completed, trade school certifications or professional license(s) earned and/or awarded, dates of attendance and/or graduation, any suspensions or revocations of licensure, and Grade Point Average.
- Professional licensure and certification verification: Verify professional license(s) earned or awarded and any suspensions or revocations of licensure. This includes, but is not limited to, registered nurses, licensed vocational nurses, social workers, certified public accountants, social workers,
- Other State Medicaid Program Exclusion Lists (list by state).
