The Vendor is required to provide a state-of-the-art automated referral application system to increase productivity.
- Response times will improve from community partners that accept patients for post-acute care services, which will allow for timely discharge of patients.
- The application will connect hospital discharge staff with providers of skilled nursing facilities, long term acute care hospitals, home health care agencies, hospice, rehabilitation agencies, and durable medical equipment agencies via the internet, using secure, and real time communication tools.
- The application will communicate with multiple Hospitals, Nursing Homes, Hospice, Home Nursing or Medical Equipment sources for review of medical conditions, and recommended treatment for appropriate care options.
- System Requirements:
• System must provide high quality providers and demonstrate the frequency in which the providers are utilized.
• Providers shall not require any subscription fees to participate.
• System must electronically reserve placement of patients, eliminating the need for phone calls, outside emails, and faxes.
• System must allow providers to electronically accept referrals.
• System must provide automated responses for receipt and acceptance of referrals from providers.
• System must identify appropriate placement for each patient by loading medical record information, complete a search throughout the country for placement facilities, home care agencies, durable medical equipment, etc.
• The search shall allow for selection of facilities and agencies based on location.
• System must utilize a timer that requires providers to respond within a specified time for the acceptance or rejection of referrals.
• System shall include the ability to handle ambulance/transportation referrals.
• System must support internal post-acute prior authorization processes from case manager registered nurse/social worker through referral to payer and response from payer.
• System shall show patients both in hard copy and online their available providers and how those providers rank against each other based on medicare, satisfaction, and readmissions.
• Must provide staff training and support prior to, during, and post implementation.
• Ability to provide a response within the established deadline of one business day or 24 hours (whichever is lesser) from time of referral to time of determination for post-acute provider to respond back with determination.
• System must be cloud based and shall not require the purchase and/or installation of new servers.
• System must allow 50 active users at one time with registered users exceeding 100 and adding 25 new usernames each year.
• System interface must be bi-directional at no additional cost to post-acute providers.
