The vendor required to provide transitional housing services (THS) and transitional housing services with programming (THS-P) for male and female individuals who are under active supervision with agency.
- Requirements for transitional housing services
a. Communication
• The THS provider will maintain open communication with local agency personnel concerning any offender who resides at their facility, including then the offender vacates the residence.
• The THS provider shall have an established communication plan that includes identifying an individual(s) who serves as the point of contact for the referral source.
• The THS provider must have the offender sign in daily and provide those logs to the agency.
b. Quality assurance
• The THS provider must participate in the agency quality assurance review, which is developed to assess the operational procedures of service providers and, if applicable, to confirm adherence to the principles of evidence-based practices.
• The THS provider must allow random site visits.
• Designated agency staff shall retain the discretion to disqualify any location for practices inconsistent with the agency philosophy and contrary to the health, safety, mental health, and physical well-being of those residing at the location.
c. Referral and eligibility requirements
• A designated agency staff member will refer offenders who meet the criteria established by the THS provider.
• Eligibility for offenders’ participation is determined by factors set by agency.
• Only offenders determined to be approved as eligible may have services paid for.
• Procedures and forms will be discussed and distributed during the post-award meeting.
• Agency is responsible for transporting the offender to the provider for initial intake; thereafter, the offender is responsible for his or her transportation.
d. Housing requirements
• Each offender shall be provided with a room that includes a bed and bedding (i.e., clean sheets, pillows, pillowcases, blankets, and bed coverings).
• Sheets and pillowcases shall be changed or cleaned at weekly intervals.
• Each resident shall be provided with personal furnishings (e.g., dresser-type storage, closet) for storing personal belongings.
• Bedrooms shall be provided with ventilation by a window, air conditioner, or other mechanical ventilation.
• Rooms with windows that open shall have insert screens, and both windows and screens must be in good repair.
• Facilities shall have adequate means of maintaining a comfortable temperature, generally 65 - 82 degrees Fahrenheit, depending upon the season of the year.
• Bathroom facilities must adequately accommodate the number of residents.
• All lavatories shall be properly ventilated.
• A furnished dining area that includes but is not limited to eating utensils, cups, napkins, a microwave, a refrigerator, and other related items.
• Facilities and grounds shall be kept clean and free from hazards to health and safety.
• Telephone services for personal use are not required, and charges associated with those services will not be covered by agency.
• Televisions, cable services, and pay-per-view are not required, and charges associated with those services will not be covered by agency.
e. Housing management
• Residence managers must maintain open communications with local agency personnel concerning any offender who resides at their facility.
• Agency agents must have access to the residence and facility.
• Failure to cooperate with agency staff concerning special requests for site visits for evaluation or to ensure compliance with the agency approved standards may result in disqualification.
f. Conflicts of interest
• Housing providers, employees, and others involved in proprietary positions with the facility, including, but not limited to, partners, family members, corporations, board members, and shareholders, shall make every effort to avoid even the appearance of a conflict of interest.
• An appearance exists when a reasonable person would conclude from the circumstances that personal interests compromised a decision or behavior of the involved person.
• Facilities owned or operated by an employee of the agency, or a spouse thereof are ineligible to become a housing provider.
