The Vendor is required to provide housing case management for households exiting homelessness to permanent housing units will utilize a housing first approach that prioritizes providing permanent housing for individuals and families transitioning out of homelessness with a housing choice voucher or rapid rehousing funds.

• The narrative must be 12-point times new roman font with 1-inch margins, double spaced and shall not exceed 8 pages.
• Whether you will be serving individuals, families, or both.
• Experience with the housing first model for housing people exiting homelessness.
• Experience providing housing case management and tenancy training
• Process for identifying housing and recruiting landlords.
• Process for inspecting and selecting new rental units.
• Procedures for conducting home visits - including the frequency
• Explain how you would engage tenants who are in violation of their lease and work with them to prevent eviction
• The type of in-kind supports that will be provided to tenants.
• Please define the duration of the wrap-around support and outline the services that will be provided.
• How you intend to interface with city staff and the community
• Provide a monthly timeline that consists of a planning phase, implementation phase and close out phase.
• The name, position, and experience of the staff members who will oversee and implement this project in-kind
• Provide an organizational chart that shows the staffing supervision and structure to oversee and operate this program.
• Include supervisors, mid-management staff, and front-line staff who will be involved in the program.
