Home and Community-Based Care Services

The Vendor is required to provide for a home and community-based system of authorized services for older adults at risk of out of home placement, allowing them to receive care in their homes or communities rather than in institutional settings.
- The Money Management program provides no fiduciary assistance to clients through activities related to managing money and effective handling of personal finances.
- Linkages. Linkages serves functionally impaired and disabled adults who are age eighteen (18) years and older, at risk of institutionalization and ineligible for other case management programs.
- Multipurpose Senior Services Program (MSSP). MSSP is a program for older adults age sixty-five (65) years and older, eligible for Medi-Cal, and at risk of institutionalization.
- Senior Options, Advocacy and Referral (SOAR). SOAR serves frail older adults, age sixty (60) years and older, who are at risk of nursing home placement and ineligible for other case management programs. The purchase of services is limited, and priority is given to minority and low-income older adults.
- Provide referred Aging & Independence Services (AIS) at the authorized frequency by AIS case manager.
- shall commence services at the scheduled appointment time. Failure to provide services at the scheduled appointment time will be considered noncompliant if this occurs without prior notice to the County.
- Services may be provided by organizations or individuals specializing in financial management or performing substitute payee functions.
- Services include, but are not limited to, the following:
• Paying bills, including assistance with online bill payment of preferred by client.
• Reconciling and rectifying bank statements.
• Organizing medical and other statements and correspondence.
• Contacting appropriate parties to collect information on all sources of income, expenses and debts.
• Providing assistance to ensure that the client is aware of financial abuse and manipulation and provide resources and assistance when abuse is discovered.
