The vendor is required to provide case management services for the purpose of assisting recipients of interim assistance with the application process.
• Conduct a comprehensive intake and review the referral materials including, but not limited to:
o SSI application history
o Documentation regarding presenting disability
o Assessment of current basic needs status
o Gather appropriate releases of information
o Establish interface protocol with health care providers (soar approach)
o Identify and reinforce current mandatory activities
o Collect and maintain all supporting documentation as necessary
• Conduct assessment of relationship with legal providers
• Establish goals and objectives with client including, but not limited to:
o Identifying retraining opportunities
o Evaluate work history and opportunities for alternate employment
o Identifying and maintaining schedule of appointments and supports to access appointments
o Actively engage client in appropriate evaluations and/or treatment
o Arrange/provide transportation to scheduled hearings
• Participate in team and case management reviews:
o Maintain regular communication with DSS and related community partners
o Communicate progress on legal, medical and other required activities
o Maintain communication with DSS to identify and disclose any activities that might immediately impact
eligibility for benefits
• Initiate appeals process as necessary, including appeals for cases not originating with the program.
• Participate in committees and task forces intended to address opportunities/community needs relevant to the population as requested by DSS.
• Provide monthly summary of cases, case types (temporary assistance for needy families (TANF) or safety net) and their status.
• Bill monthly in accordance with the attached budget, specifying the numbers of hours worked.
• Provide a year-end report that details the number of individuals served at what stage (application, appeal, etc.) and a narrative about the challenges of serving this population and suggestions for improving service.

• Provide monthly the number of new clients assessed
• Provide monthly the number of clients being provided with case management services
• Provide monthly the number of SSI applications submitted; the number of SSI applications pending; the number of applications approved/denied; the number of appeals filed; and the status of any appeal pending
• Keep a case record (narrative) of direct contact with clients and with any collateral contacts.
