The vendor is required to provide the case management services consist of specific types of activities designed to develop, accomplish and periodically update an individualized service plan that addresses the participant’s needs related to food, clothing, housing, health care, health insurance, substance use dependency, physical health, mental health or related issues: screening, assessment, case coordination, partnering with department staff, and strategic community partnerships.
- Provide a 24/7 access phone number to receive referrals and take calls from law enforcement.
- While a physical response is not always necessary outside of business hours, it may be requested.
- Calls are regularly used for new referrals, of which there have been about 80-90 per year.
- Helping participants gain access to funding programs (e.g., temporary assistance, veterans’ assistance, social security).
- Helping participants find and maintain housing, if that is one of their goals.
- Assisting participants in identifying and pursuing legal means of income.
- Using an electronic health record or data system that allows for regular sharing of identified data and service records with project management.
- Part of harm reduction requires engaging directly with people who use drugs “to prevent overdose and infectious disease transmission; improve physical, mental, and social well-being; and offer low barrier options for accessing health care services, including substance use and mental health disorder treatment.”
- Along with a small client outreach and engagement team, will be responsible for meeting with people who have been sent to department by eligible referral sources.
