The Vendor is required to provide traditional care coordination services play a crucial role in ensuring that children, youth, and their families receive comprehensive behavioral health services and support.
- By providing individualized support and guidance, care coordination services help navigate complex systems, connect families with appropriate providers, and ensure that services are tailored to meet unique needs. 
- This holistic approach aims to empower families, enhance coping mechanisms, and promote long-term wellbeing for the youth. 
- The integration of various service components, such as family peer support services, as well as customized goods and services, further strengthens the support network available to families, allowing for a more comprehensive and effective care experience.
- Targeted Case Management Plus (TCM+) is a community-based behavioral health service designed to expand access to youth care coordination and family peer support services statewide, with a focus on families with private insurance or those not eligible for Medical Assistance.
- Responsibilities for agency shall include outreach and engagement strategies for children and youth at risk of serious emotional and behavioral challenges, with the goals of increasing awareness of available services, promoting family resilience and stability, and facilitating access to appropriate care.
