The Vendor is required to provide case management services to clients referred by agency.
- Service include:
• Provide ongoing case management, housing stability, and tenancy support for families referred by agency who are low income and at risk of housing instability or homelessness;
• The number of clients served will be dependent on how many clients choose to take advantage of services; these case management services will be made available to participants in other agency programs, which will be roughly 80-100 clients;
• Through these services, the applicant will identify supports, employment/income opportunities, housing retention barriers, and work to resolve issues or conflicts that may lead to tenancy problems;
• Provide budgeting, life skills support, housing stabilization, goal planning and revision, workforce development referrals, and onsite support;
• Provide referrals as needed for behavioral health, mental health, and physical health; and
• Provide regular reports to agency on funds spent, client interactions, and client outcomes.
- Budget for these services of a maximum of $120,000.
