The Vendor is required to provide special child health case management is a goal-directed system.
- Provide coordinated case management services, including but not limited to:
• Case management services via telephonic, virtual, home visitation, or on-site meetings;
• Child and parent counseling;
• Identifying the child’s problems and needs;
• Validating parental knowledge and understanding of the child’s problems and needs;
• Assessing the need for services;
• Coordinating the child and parent to medical rehabilitation, educational, social, and financial assistance;
• Child find and referral
• Providing health plans to assure health promotion efforts that include primary prevention, early identification, evaluation and treatment
• Communicable disease prevention and control
• Establishing and maintaining written procedures and protocols
• Communicating with clients and families whose native language is not English.
• Either written or verbal translation must be provided, as appropriate, including conversations, meetings, individual services plans, letters to the family and other documents as needed.
• Parent-to-parent support;
• Respite and child care services;
• Family training and counseling;
• Support groups;
• Sibling groups;
• Resource information; and
• Referrals as appropriate.
