The Vendor is required to provide to social service providers to deliver the needs of the Social Services Referral and Case Management Program
- The Social Service Referral and Case Management Program will take advantage of the unique experiences of City staff and affiliated program staff that will help to link at-risk residents to needed social services.
- The ultimate goal of contracting a community-based organization to manage referrals is to reduce health and safety risks by providing the City early awareness of identified problems and expanding staff capacity to respond with both codes enforcement and support services interventions, and to reduce the need for repeated interventions while providing the City with up to date information on the overall social needs of the community.
- Program implementation will include data collection on the number of referrals given, referrals accepted or denied, services provided, case duration, and outcomes.
- The inspector and/or staff person issues the referral either by directly connecting the resident to the dedicated case manager or by alerting the case manager of the property at which service(s) are needed.
- Outreach will be supported by, for example, Inspectors will leave informational brochures detailing the types of services available and how to contact the community-based organization at every residence inspected.
- The case manager follows up with the resident after reviewing information about the referral and the property to determine if, what type, and where a referral should be made.
- These risks include, but are not limited to:
• Substance use disorders
• Emotional, mental, and/or physical distress or abuse
• Housing insecurity and rental assistance
• Eviction
• Need for relocation
• Food insecurity
• Hoarding
• Need for access to healthcare and/or to mental health services
• Need for fuel assistance and/or conservation
• Need for access to care for the elderly
• Need for access to childcare
• Responsible payee
• Need for Tax preparation
• Need for Weatherization
- Management and case managers of the responding agency should have proficiency (verbal and reading) in Spanish and have demonstrated experience working with individuals with mental health issues, substance use disorders, and economic hardships.
