The Vendor is required to provide asthma case management capacity building services for include:
• The applicant must submit a project narrative outlining how they will support capacity building for school nurse-led asthma case management to improve outcomes for students with asthma.
• Asthma case management is a collaborative approach to improving asthma outcomes through development of individualized health care plans for children with asthma that have measurable goals; implementation of activities to meet those goals; and tracking of relevant outcomes
• Capacity building for identification of students with asthma.
• Development of a systematic method to identify and track information about all students with asthma.
• Determination of criteria for designating students to receive school nurse-led asthma case management
- Capacity building for care planning and outcomes evaluation
• Development of individualized health plan (IHP) templates for students with asthma that include measurable goals in specific areas to facilitate outcomes tracking.
• Goals and related outcome areas should include, but are not limited to:
• Increase in asthma self-management skills
• Decrease in asthma exacerbations
• Decrease in school absences
• Creation of a mechanism for routine IHP data collection and basic analysis of outcomes
• Development of data sharing agreements as needed for access to student data such as attendance.
- Pilot testing of school nurse-led asthma case management in a minimum of one elementary and one secondary school to refine processes.
