The Vendor is required to provide intensive case management services for include:
- Engagement and case management
• Carry a defined caseload of department referred households and conduct comprehensive, holistic assessments identifying strengths, needs, and opportunities to reduce reliance on public assistance.
• Workers will co-develop individualized service plans focused on housing stability, income growth, and long-term economic independence.
• Workers will maintain regular, supportive contact as the primary point of connection, provide advocacy with landlords, employers, and service providers, and use flexible, innovative engagement strategies to maintain participant connection.
- Housing stability and diversion activities
• Implement strategies to prevent homelessness or emergency placement, assist with housing searches and applications, and support budgeting and landlord engagement, and provide assistance with move-in logistics and tenancy education.
• Creative housing solutions may be used when traditional options are limited.
- Collaboration requirements
• Participate in case conferences, team meetings, and cross-system coordination as requested by department.
• Maintain strong working relationships with landlords, community providers, and workforce partners and communicate regularly with department staff regarding participant progress, barriers, and emerging needs.
• Retains authority for case direction and will issue directives that the contractor shall carry out to ensure alignment with department policy, cross-system coordination, and program goals.
