Reporting Services

The Vendor is required to provide internal revenue service (“IRS”) 1095-c/1094-c reporting services (“services”), for the county’s employee medical health plan (“EMHP”).
- Services include:
• Perform Services in accordance with the Federal guidelines and as directed by the County.
• Schedule a discovery meeting with the Department, subject to the Department's designee’s availability.
• Prepare a complete discovery document to gather County rules/requirements and obtain the County’s approval.
• Provide a detailed standard file layout applicable to the identified data sources.
• Load data from multiple sources.
• Configure business rules to interpret the discovery data.
• Aggregate any County group data for control group consolidation.
• Map demographic and benefit information into the Contractor's proprietary system.
• Test initial import files and complete data audits to ensure the required data is provided and perform a minimum of two (2) retests.
• Load initial import file and provide any applicable error reports and missing data reports to the County.
• Derive applicable codes based on the information provided by the County and/or file vendor(s).
• Calculate and maintain the employee counts for Form 1094-C reporting.
• Maintain transition relief data, if applicable.
• Provide ongoing compliance consulting on County reporting changes.
• Provide an annual web-based session of training for the County’s employees on system functionality.
• Provide the County with a toll-free phone number and provide a call center for County inquiry support. State the hours of operation for the call center.
• Provide IRS inquiry support regarding reporting provided to the IRS when requested.
• Print, fulfill, and distribute Form 1095-C following IRS published procedures.
• Initial transmittal of Form 1094-C on behalf of the County following IRS published procedures.
• Update and/or modify the discovery document upon e-mail notification by the County.
• Provide a final copy of the discovery document to the County, in electronic form, prior to any draft reports.
• Provide true copies of the 1095-C Forms in electronic form prior to fulfillment.
