The vendor is required to provide the integrated behavioral health services program at the central receiving division will implement and operate a program for the delivery of integrated behavioral health services for individuals who are experiencing mental health, substance use, and medical challenges.

a. Screening and assessment
• Perform medication reconciliation to ensure accurate continuation of prescribed medications.
• Provide medication management, including administration and monitoring.
• Conduct assessments for medication-assisted treatment (mat) and medications for opioid use disorder (MOUD), and provide treatment as needed.
b. Crisis intervention
• Deliver urgent or crisis mental health and medical interventions to stabilize individuals in acute distress.
• Ensure timely response to behavioral health crises within the division.
c. Collaboration and continuity of care
• Collaborate with the jail diversion program to support alternatives to incarceration for eligible individuals.
• Notify the division of individuals requiring ongoing mental health support.
• Facilitate linkage to crisis support management and other supportive services to ensure continuity of care post-release.
d. Sustainability and funding
• Collaborate with division on funding models, including potential reimbursement through Medicaid, medicare, and private insurance.
• Project and takes steps to progress towards long-term self-sustainability of integrated behavioral health services at the division.
e. Performance metrics and evaluation
• Design and implement key performance indicators (KPIs) to evaluate the success of the integrated behavioral health services program at division.
