Mental Health Therapy and Targeted Case Management Services

The Vendor is required to provide mental health therapy and targeted case management services will be provided on-site during the school day and aligned to the fps academic calendar.
- Mental health therapy services
• Deliver in-person, school-based therapy aligned to the fps calendar
• Provide:
o Diagnostic assessments
o Individual therapy
o Group therapy
o Family engagement as appropriate
• Utilize evidence-based practices such as trauma-informed care, CBT, and DBT
• Participate in school-based team meetings
• Coordinate with school staff to minimize disruption to instruction
• Manage billing (insurance and Medicaid) independently
- Targeted case management (TCM)
• Deliver targeted case management services in accordance with state guidelines
• Support:
o Care coordination across systems (school, family, community providers)
o Service planning and monitoring
o Resource connection and follow-through
• Participate in school team meetings and planning processes
• Maintain required documentation and compliance
- Combined services (preferred)
• Proposals offering both services will receive additional evaluation points
• If separate providers are selected:
o Agencies must demonstrate a formal partnership and coordination plan
o Providers must function as integrated members of the school team.
