Occupational Health Services

The Vendor is required to provide occupational health services for include:
- Pre-employment exams
• All physicals include audiometric testing and basic eye exams.
• Physicals are conducted for all full-time employees (white-collar and clerical positions, blue-collar and labor positions, and firefighter and first aid members); pre-employment.
• Advice township officials as to possible reasonable accommodations when an individual has an inability or limitation in performing the essential functions of the job.
• Comprehensive eye exams shall include peripheral vision, stereopsis, color vision and visual acuity.
- Asbestos exam
• Asbestos exams shall include history and physical exam, osha screening questionnaire review, chest x-ray pa and lateral and pulmonary function test.
• Bloodwork may be performed if clinically indicated.
- Hazmat exams
1. Pre-placement and termination hazmat exams
• Medical and occupational history and physical exam
• Review of systems questionnaire
• Audiogram and snellen vision testing and ishihara color vision testing
• Urinalysis
• Spirometry pulmonary function test
• Baseline chest x-ray
• Baseline ekg
• Complete blood count
• Basic metabolic panel
2. Annual hazmat exams
• Medical and occupational history and physical exam
• Review of systems questionnaire
• Audiogram and snellen vision testing and ishihara color vision testing
• Urinalysis
• Spirometry pulmonary function test
• Complete blood count
• Basic metabolic panel
3. Post exposure hazmat exams
• Interim medical and occupational history and physical exam
• Review of systems questionnaire
• Testing as dictated by symptoms and type of substance exposed to as an additional cost. Additional costs shall be itemized on the invoice.
- Workers compensation services
• Repetitive motion injuries
• Slip and fall
• Head, back and neck injuries
• Broken bones
• Cuts
• Burns
• Sprains and pulled muscles
• Contact with objects and equipment or entanglement
• Motor vehicle accident
• Exposure to harmful substances or environments
• Electrocution.
