The Vendor is required public health consulting services on an as-need basis.
• Services may include, but are not limited to:
• Community Health Assessment (CHA) development
• Community Health Improvement Plan (CHIP) development
• Strategic planning
• Grant research and identification
• Grant application development and submission support
• Grant administration and compliance monitoring
• Performance measure tracking and reporting
• Program evaluation and outcome measurement
• Public health data analysis
• Community engagement and stakeholder facilitation
• Needs assessments and gap analyses
• Federal and State grant reporting
• Public health accreditation support
• Workforce development planning
• Other public health planning and grant-related consulting services
