The Vendor is required to provide Employment and Income Verification (EIV) Services data, to be used in performing eligibility determinations for applications, renewals, and continued eligibility and benefit level for government benefits.
- An EIV Inquiry by an Authorized User shall provide the following information, to the extent the information has been provided to the Contractor by its employer clients, and without requiring the subject of the verification to provide access or log in to their employer(s) or bank(s) systems:
• Employee Name
• Employee Date of Birth
• Employee Social Security Number (SSN)
• Employer Name
• Employee Address
• Employee Phone Number
• Employment Status
• Employment Dates, including most recent hire date and termination date (if terminated)
• Pay Rate(s)
• Gross Earnings, including tips and bonuses.
• Pay Frequency (e.g. hourly)
• Pay Cycle (e.g. monthly)
• Pay Dates
• Number of Hours Worked per Pay Period
• All available current and historical year to date gross and net income details, including first and last pay dates and intermittent pay history.
• Phone Number for the Custodian of Records for the Employer
• Position Title
• Reason for Termination, if terminated
• Medical Insurance Information
• Length of Employment
• Yearly Gross Income
- Scheduled downtime and/or maintenance windows that result in an outage, unavailability, or interruption to the Web Portal, Batch, or API services or data shall be reported in advance to agency.
