Athletic and Student Accident Insurance Services

The Vendor is required to provide Athletic/UIL and Student Accident Insurance coverage for agency.
- Benefit expectations:
• Heat exhaustion and heat stroke shall be treated as a covered accident.
• Dental coverage shall be included as part of the plan benefits.
• Accidental Death and Dismemberment (AD&D) coverage shall be included.
• Participation in a Preferred Provider Organization (PPO) network is strongly preferred.
• The District requires detailed loss run reports for all claims submitted. Monthly reporting is preferred. A sample copy of the proposed loss run report shall be included with the proposal.
• A sample policy for each insurance carrier included in the proposal shall be provided.
• A sample claim form, including filing instructions for each policy, shall be included.
• A copy of the bidder's current insurance license shall be provided.
• Proof of the bidder's current Errors and Omissions (E&O) insurance coverage shall be included.
