The vendor is required to provide medical surveillance program, as administered by the office.
- Provide occupational medical evaluation and examination services:
• Asbestos medical screening (2,700 employees)
• Respirator medical examinations (2,700 employees)
• Blood lead and zinc protoporphyrin (ZPP) levels testing (500 employees)
• Medical monitoring for exposure to hazardous materials (60 employees)
• Audiometric testing (1,300 employees)
• Employment medical screening for the use of respirators (as required)
- Provide a minimum of one (1) permanent facility to provide clinical appointments.
- The clinic must be open during regular business hours 8:00 a.m. to 5:00 p.m. and located within a 50-mile distance of district boundaries.
- Coordinate clinic appointments within five (5) working days of request from classified personnel assignments.
- Provide specialty tests to include, but not limited to, CT scans, new hire pre-employment screenings, and TB skin tests scheduled by personnel.
- Medical screening services to include, but not limited to, physical exam, spirometry test, x-rays, blood draw for blood lead and zinc protoporphyrin blood (ZPP) tests, audiometric testing, and vital sign check such as weight and blood pressure testing.
- Standard x-rays, TB skin tests and audiometric testing shall be conducted in the mobile unit.
- Mobile unit screenings will take place at various district sites for four to five (4-5) days per month, depending on the district’s needs.
- Provide an alphabetical list of employees due for medical screening that includes employee identification numbers, dates of birth, work locations and job classifications for each screening location at least five (5) days in advance of the screening date.
- Provide a licensed physician that is board certified in occupational medicine to provide directions to employees who are required to wear a negative pressure respirator while performing their work.
- Respirators currently used consist of negative pressure air-purifying respirators, powered air purifying respirators and self-contained breathing apparatus.
- A medical and work history with special emphasis directed to the pulmonary, cardiovascular, and gastrointestinal systems.
- Blood pressure and cardiac status enabling individuals to perform vigorous activity.
- Thorough physical examination with particular attention to teeth, gums, hematologic gastrointestinal, renal, cardiovascular and neurological systems.
